A screw picked up near Heathrow at J14 or a nail on the Hertfordshire stretch is often repairable, provided you leave the M25 before the tyre runs soft and the hole sits in the central tread.
The orbital collects debris from every building site and freight yard it passes, and the pressure warning tends to light somewhere awkward, perhaps between J12 and J13 where the M3 interchange offers nowhere to pull in. What decides the outcome is the distance covered afterwards. A tyre driven several junctions while deflating chews its inner liner, and by then the repair question has been settled against you before anyone has looked at it.
With the car parked, the wheel is unbolted and the tyre prised from its rim, so the liner can be examined. A hole of up to 6 mm, sitting within the central band that covers three-quarters of the tread, receives a combined patch-plug fitted from within to BS AU 159. Damage on the shoulder, a sidewall cut or two holes sitting too close together means a replacement instead. The price for either outcome is set out on the call.

Park at Cobham or South Mimms, or leave at J13 for Egham, J17 for Maple Cross or J26 for Waltham Abbey and stop in a car park there. Repairs are off limits on a slip road, the orbital's hard shoulder and its emergency areas.

A slow leak picked up on the orbital often shows itself later, perhaps as a soft front tyre spotted at Tower Retail Park after leaving at J1, outside Crayford station, or on a Barnehurst driveway the next morning. Leave the car where it is and send a photo of the tread. Where the hole falls within the limits set by BS AU 159, the tyre gets plugged and patched right there.
Parked up at Cobham or a Chertsey retail car park off J11, the punctured corner should not turn another wheel. That can of foam in the boot is tempting, but squirting it in leaves the fitter a gummed-up liner and sometimes a tyre that can no longer take a patch. Crouch down and find the screw: dead centre is hopeful, out near the edge much less so. Meanwhile the locking nut key needs finding, so the Cobham visit is not held up by its absence.
